ADMISSION REQUIREMENTS FOR HIGH SCHOOL GRADUATES

1. Form 138 (Report Card)
2. Certificate of Good Moral Character from School previously attended
3. 4 (2x2) pictures
4. 2 (1x1) pictures
5. Birth Certificate from NSO (photocopy)
6. NCAE result or its equivalent (photocopy) to be submitted at the Guidance Office
7. IQ test - to be taken at the Guidance and Counseling Center
8. Medical/Physical examination - University Clinic

FOR TRANSFEREES

1. Transfer Credential (Honorable Dismissal) Signed by the Registrar and duly sealed.
2. Certificate of grades for evaluation
3. Certificate of good moral character signed by the
Dean and the Guidance office with school seal
4. 4 (2x2) pictures
5. 2 (1x1) pictures
6. IQ test administered by the Guidance and Counseling Center
7. Medical/Physical Examination - University clinic

FOR CROSS ENROLEES

1. Permit to cross enroll from the student's mother school
signed by the Registrar.
2. Four copies (2x2) photo
