ADMISSION REQUIREMENTS FOR HIGH SCHOOL GRADUATES

1. Form 138 (Report Card)
2. Certificate of Good Moral Character from School previously attended
3. 4 (2x2) pictures
4. 2 (1x1) pictures
5. Birth Certificate from PSA (photocopy)
6. NCAE result or its equivalent (photocopy) to be submitted at the Guidance Office
7. IQ test - to be taken at the Guidance and Counseling Center
8. Medical/Physical examination - University Clinic

FOR TRANSFEREES

1. Transfer Credential (Honorable Dismissal) Signed by the Registrar and duly sealed.
2. Certificate of grades for evaluation
3. Certificate of good moral character signed by the Dean and the Guidance office with school seal
4. 4 (2x2) pictures
5. 2 (1x1) pictures
6. IQ test administered by the Guidance and Counseling Center
7. Medical/Physical Examination - University clinic

FOR CROSS ENROLEES

1. Permit to cross enroll from the student's mother school signed by the Registrar.
2. Four copies (2x2) photo

Foreign Students

ADMISSION REQUIREMENTS FOR FOREIGN STUDENTS

1. Letter of application

2. Valid passport

3. Alien Certificate of Registration (ACR)

4. Certificate of Registration of Temporary Student (CRTS)

5. Original Transcript of Records duly authenticated by the Philippine Embassy or Consulate in the country of origin

6. Certificate of Eligibility of Admission (CEA-Medicine, Dentistry, from student services, CHED, Manila)

7. Police clearance duly authenticated by the Philippine Embassy or Consulate from the country of origin

8. Two (2) copies Certificates of Good Moral Character from a professor and another One preferably from the Dean/Principal

9. Affidavit of financial support accompanied by Bank Statement

10. Personal history statement duly accomplished in five (5) copies with fresh signature and original passport photo in each page

11. Six (6) copies of signed passport size pictures

12. Aptitude Examination administered by the VMUF Guidance and Counseling Center

13. Medical/Physical Examination by the University Clinic

Medicine

ADMISSION REQUIREMENTS FOR MEDICINE

1. Letter of application with the following documents

a. 2 copies of signed passport size photo
b. Baccalaureate degree diploma in Pre-Med course. If graduation is at end of the second semester before the date of application, a certificate for the expected completion by the Dean of that School is needed
c. Official Transcript of Records
d. Police Clearance certificate
e. Two (2) certificates of good moral character from the Dean and Guidance Office/Professor
f. Birth Certificate (photocopy)

2. Applicants must fill out completely and accurately an application form in the administration office

3. Application fee of Php. 300.00 which is not refundable

4. NMAT Rating

5. Certificate of Eligibility for Admission (CEA) from the Commission on Higher Education, Manila

6. Interview by a member of the Admission Committee

7. Aptitude Test

Dentistry and Veterinary Medicine

ADMISSION REQUIREMENTS FOR DENTISTRY AND VETERINARY MEDICINE

1. OTR/Certification of Grades

2. Certificate of Eligibility for Admission form the Admissions office, VMUF duly signed by the Dean, Registrar and President

3. Two (2) certificates of Good Moral Character

4. Two (2) copies of passport size picture

5. Birth Certificate (photocopy)

6. Mental ability test administered by VMUF Guidance and Testing Center

7. Medical/Physical examination - University Clinic

Law

ADMISSION REQUIREMENTS FOR LAW

1. Must be a Filipino citizen

2. A holder of any Bachelor of Science or Arts degree, preferably with major in English, Logic, Political Science, History or Economics and must have at least 18 units in English, 6 units in Mathematics and 18 units in Social Sciences

3. Official Transcript of Records (OTR)

4. Diploma

5. Birth Certificate in Security Paper (PSA)

6. Two (2) 2x2 pictures with white background

7. Certificate of Eligibility for Admission from the Admission Office duly signed by the Registrar and President

8. Certificate of Good Moral Character from two (2) reputable persons

9. Medical/Physical examination - university clinic

10. A non-refundable Admission Fee

Graduate School

ADMISSION REQUIREMENTS FOR GRADUATE SCHOOL

1. Transfer Credential/Official Transcript of Records

2. For admission to the Masteral's Program - a Baccalaureate degree with a weighted average at least 85% or B or 2 for the entire course

3. For admission to the Doctoral Program - Masteral's degree, a weighted average of at least 88% for the entire course

4. In certain specific fields of concentration the corresponding undergraduate academic background or 12 units in the same discipline at either undergraduate or graduate level before admission to the degree program

5. Pass interview of a member of the Admission Committee

• Students with a lower general average than the prescribed weighted average may be probationally admitted to the Graduate Programs if they demonstrate the capacity to undertake said programs, may be formally admitted upon the recommendation of the Admission Committee with the approval of the Dean
