ENROLMENT PROCEDURE
Step I
College / Department
- Fill out Student Basic Info Slip
- Fill out Tentative Student Load Sheet
- Advising and Scheduling of subjects to take
- Signing of the Tentative Student Load Sheet
Step II
Business Office (1st floor VM Centrum building - Windows 3, 4, 5, 6, 7, and 8)
Payment and Assessment
- Present your Tentative Student Load
- Payment of Application Fee and Tuition Fee
- Get your printed OFFICIAL ENROLLMENT SHEET with Official Receipt
Step III
Registrar (1st floor Centrum Bldg) Staff-In-Charge
- Submission of official documents / credentials
- Get your permanent ID no.
- Stamping of Official Enrollment Sheet with OFFICIALLY ENROLLED and affixing signature after having checked and received all admission requirements
Step IV
Office of the Student Affairs (1st floor BA Bldg.)
- Fill out GSPIS (General Student Personal Information Sheet)
- Submit your Official Enrollment Sheet (Registrar's Copy)
Step V
Educational Media Center (2nd floor VM Centrum Building)
- Picture Taking for VMUF Digital ID
- Get your VMUF ID
END OF ENROLLMENT
Note:
- IQ Test (Guidance Office)
- Library card (Library Office)
- Medical Exam (Virgen Milagrosa Medical Center / University Clinic & payment will be scheduled by the Department Heads and Medical Staffs)
- All Official Receipts & Official Enrollment Sheet (Students's Copy) must be presented.
